[B] Scope of Service:
(1) Vendor needs to provide for screening and care management services for home-based care system to assist older persons to exercise self-determination and to remain as independent as possible, assessment referral and care management services to eligible clients, information and assistance about aging concerns, services and the aging network to the government authority located in Cincinnati, OH.
(2) All the question must be submitted no later than December 1, 2017.
(3) A contract term will be for three years.
