[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide infant hearing family outreach services to Develop and submit for ODH approval an outreach contact communication plan for contacting families by phone without use of robot calls or other automated contact methods at nine weeks after baby’s birth. Timing of outreach is identified in the ODH tracking and follow up system and through the ODH report for families whose child has not received a complete and confirmed diagnosis. Plan shall be structured to support all activities in a plan, do, study, act approach. Goals, strategies, objectives, activities, evaluation methods and outcome measures to coordinate offering of family services that align with state and local systems including Provide contact information for paediatric audiologists, provide information regarding importance of diagnostic hearing evaluations following non-pass screening results, encourage families to schedule and attend appointments, provide resources to parents to assist in overcoming barriers to appointment, Provide diagnosis specific resources pathways to parents whose child has been diagnosed with middle ear fluid. development of a state-wide script that will be used when offering services to families, commitment to reporting of data (date of call, outcome of outreach efforts, additional information as requested in the approved script for contacting families) from each call into the odh case management system within four business days of occurrence, ensure enrolment in part c early intervention, inform families of all communication methods in an objective and unbiased manner, development of a state-wide script that will be used when offering services to families.
