Vendor needs to provide medical and life insurance services must include the following submitted rates including length of rate guarantee, implementation credit (if applicable), cover letter, service team’s names, roles, and professional experience, office/location which services the account, customer service hours of operation, claims filing process, administrative options available to employer (i.e. Self-bill, list bill, etc.), evidence of insurability submission process (i.e. online, paper, web portal), availability of and access to electronic reporting, description of value-added services, if applicable, implementation process and timeline, all required forms as indicated in the bid packet, any plan design deviations must be clearly identified to the government authority located in Cleveland, OH.

- Onshore (USA Only);

Performance of the work will be Offsite. Vendor needs to carry work in their office location
