[A] Budget: Looking For Proposal
[B] Scope of Service:
(1) Vendor needs to provide ending the HIV epidemic health navigators and support services to the government authority located in Columbus, OH.
- To support HIV retention in care among persons at highest risk for not being retained in medical care or not being virally suppressed,
- Conversations with communities impacted by HIV and promising national practices indicate using Health Navigators increases the likelihood of persons to initiate and remain in medical care, as well as achieve viral suppression,
- Sub-recipients will be responsible for hiring or accessing Health Navigators, a role most similar to Peer Navigators, but HIV status neutral. Health Navigators will be individuals from the community who authentically understand social determinants of health and can navigate the health system from personal experience.
(2) A conference call will be held on May 6, 2020.
(3) All questions must be submitted no later than May 14, 2020.
[C] Eligibility:
Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
