[A] Budget: Looking for proposal
[B] Scope of Service:
Vendor needs to provide health care services for geriatric, medical, mental health, and employee health care to the government authority located in Ohio.
- Inpatient Care: Contractor’s NP(s) shall provide geriatric, medical and/or mental health inpatient services for 5 Veteran Community Living Center units: One Hospice/Palliative Care unit; Two Geriatric Psych units, one Dementia and one Skilled Care Rehabilitation; and Two Mental Health inpatient units, which includes Acute Psych and Chronic Psych.  Contractor’s nurse practitioner(s)shall be present and on time at the scheduled start time.   
- Outpatient Residential Care: Contractor’s nurse practitioner(s) shall provide geriatric, medical, and/or mental health services to the Substance Abuse Residential Recovery Treatment Program
- Employee Health Care: Contractor’s nurse practitioner(s) shall provide employee health care services.
- Employee Health Care: Contractor’s nurse practitioner(s) shall provide employee health care services.
- Medications: Contractor’s nurse practitioner(s)shall follow all established medication policies and procedures. No sample medications shall be provided to patients.
- Discharge education: Contractor’s nurse practitioner(s)shall provide discharge education and follow up instructions that are coordinated with the next care setting

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
