[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide services consistent with professional standards of care and comply with all medical and ethical requirements on an as needed basis to the government authority located in Ohio.
- Coordinate and manage mandated vision and hearing screenings. Provide follow-up as required. Perform re-checks on all students failing initial vision screenings
- Send referrals letters to parents of all students who fail any mandated screening.
- Conduct health assessments for students, providing appropriate referrals and follow-ups as needed with parents and heath care providers for student with injuries, illnesses, and potentially handicapping conditions, or children having recurrent complaints of frequent absences.
- Referral and follow up of screening failures including facilitation of school-based care for dental, vision and medical services.
- Case management for children with chronic illnesses through parent, health professionals and school staff collaboration to develop Individualized Health Plans, ensuring Emergency Action Plans are in place for student safety, coordinating care with physician through medical orders, staff training and daily treatment and procedures.
- Chronic Illness can impact the child's ability to learn or function within the classroom setting and present a significant threat to the child's well-being, attendance and success in school.
- Daily administration of medications Inventory and safely secure all medications
- Coordinate with parents and authorized health care provider orders for administration of medication, effectiveness, performance, possible side effects and special training needs.
(2) All the questions must be submitted no later than June 1, 2020
(3) The contract period will be for one year.
