Vendor needs to provide cancer control program to populate diagnosis and treatment data on cancer survivorship care plans with information from electronic health records in a selected hospital or health system located in a high need/priority area of the state, as defined by the comprehensive cancer control program, solution that shall have the ability to develop procedures within an identified commission on cancer accredited hospital in a high need area of the state that will move clinical data between health care providers from the EHR to the cancer survivorship care plan software and incorporate this into the template for the survivorship care plan to the government authority located in Columbus, OH.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location
