[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide peer support all services will be provided by the community support network working with multi-cultural adult populations to the government authority located in Columbus, Ohio.
- Implements patient satisfaction surveys and enters information into a data base,
- Provides Peer support 1 to 1 with patients who have been referred by treatment teams,
- Speaks to new employees, student nurses, student doctors and student social workers
about the Peer Program and life's experiences having a mental illness,
- Provides a weekly report concerning work activities to supervisor.
(2) The contract term will be for two years.
[C] Eligibility:
- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
