[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide occupational health services including but not limited to pre-employment, return-to-work, and fitness for duty examinations; drug and alcohol testing for covered DOT and non-DOT employees; industrial injury care; respirator physicals to provide with medical consultation for approximately 750 employees,
- Conduct a pre-employment physical examination for all potential employees to determine the ability to perform the essential functions of the position; examining candidates to determine limitations, impediments, or pre-existing conditions which would affect their ability to perform the essential functions of the position with/without a reasonable accommodation,
- Conduct initial physical examination of employees following an accident, which occurred during the
course of employment, and render treatment, where appropriate, as the designated treating physician unless the employee to the government authority located in Cleveland, OH.
(2) All questions must be submitted no later than September 12, 2019.
(3) A contract term will be for two years.
